Eight required documents — all must be current and complete

Driver Application for Employment Fully completed and signed, capturing all required history and disclosures
Motor Vehicle Record (MVR) From each state where the driver held a license in the past 3 years — reviewed annually
Road Test Certificate or Equivalent Completed road test form or accepted CDL documentation per §391.33
Medical Examiner's Certificate Valid DOT physical, including any required waivers or exemptions
Safety Performance History Records Verified from previous employers per §391.23, with documented driver rights to review and rebut
Annual Driver Review Documented evaluation of driving performance and updated MVR
Driver's License Copy Current and valid, matching the type of vehicle operated
Drug & Alcohol Testing Records If applicable — including pre-employment screening and consortium enrollment
